Andrew Scull noted Sakel's claim: "He claimed that this Insulin Coma Treatment cured eighty percent of his -- eighty percent tends to come up again and again in these treatments as a sort of percentage that you cure."

Dr. Ladislas Meduna wrongly believed that schizophrenia and epilepsy were mutually exclusive, leading to the idea that inducing an epileptic seizure could 'drive out' schizophrenia. Early attempts with camphor and Metrazol caused violent seizures and frequent bone fractures, necessitating a search for less brutal methods.

Italian psychiatrists Cerletti and Bini observed pig slaughterhouse practices to refine electroshock techniques, transitioning to direct electrical stimulation. Initially, ECT was often used punitively to control patient behavior rather than as a therapeutic intervention, reflecting the primitive understanding of mental illness.

Andrew Scull describes Meduna's flawed logic: "If you had epilepsy, you didn't have schizophrenia. If you had schizophrenia, you couldn't have epilepsy... So then, the logical next step was, 'Well, if we could create an artificial epileptic seizure, maybe we would drive out the schizophrenia.'"

The modernization of electroconvulsive therapy (ECT) successfully eliminated physical trauma, but significant ethical controversies persist. The introduction of muscle relaxants largely ended the problem of fractures caused by seizures, making the procedure physically safer. However, this also necessitated anesthesiology support due to the respiratory risks associated with muscle paralysis.

Despite these advancements, critics continue to argue that ECT causes severe memory loss and potential brain damage. It remains a controversial last-resort intervention, typically reserved for treatment-resistant, suicidal depression, highlighting ongoing debates about its long-term effects and appropriateness.

Andrew Scull explained the critical improvement: "What changed ECT a bit, quite a bit actually, was giving muscle relaxants so that people didn't thrash about, and the fractures were largely a thing of the past."

This mistrust results in aborted clinical trials and the loss of critical knowledge that could otherwise advance treatment. Compounding this issue are funding cuts and training gaps for new scientists, further hindering research and development.

Andrew Scull warned about the severe consequences of this erosion: "After four years of this, first of all, the degree of mistrust will have grown exponentially, and once trust is lost, it's very hard to recover. Secondly, the science itself is being destroyed."

Genome-wide association studies have failed to identify simple Mendelian genes that would definitively explain conditions like schizophrenia or bipolar disorder. Renamed a 'major tranquilizer' and later an 'antipsychotic,' chlorpromazine fundamentally shifted mental hospital management toward chemical sedation, profoundly impacting patient care and institutional practices.

SmithKline & French transformed from a small company into a major player largely due to the success of Thorazine. They employed compelling visual aids, such as moving pictures of agitated patients becoming calm after medication, to sway state politicians.

This strategy led to widespread adoption of chemical treatments and spurred the emergence of numerous copycat drugs, initiating the boom in minor tranquilizers. These marketing efforts fundamentally shaped the landscape of psychiatric care and medication usage.

Andrew Scull described the marketing approach: "The companies wake up, they sell it to the politicians. They go to the state legislatures. They have moving pictures of an agitated patient who turns calm, you know."

Early antipsychotics caused severe long-term iatrogenic damage, particularly tardive dyskinesia, a condition that was largely ignored by the medical profession for two decades. Patients developed incurable jerky movements and facial twitching, often mistaken for symptoms of mental illness itself. If someone exhibiting these symptoms walked down the street, they would be perceived as mentally ill, when in reality, they were suffering from the side effects of their medication.

The condition is called 'tardive' because it develops late after treatment initiation, and 'dyskinesia' refers to the uncontrolled body movements. It was only after George Crane's paper in Science finally brought the issue to light that the medical community began to acknowledge this devastating consequence of psychiatric drugs.

Andrew Scull explained: "Tardive because it's late developing... and dyskinesia because what involves is jerky uncontrolled movements of the body... If somebody like that is walking down the street towards you, you will think there's a mentally ill person... what you're seeing is the iatrogenic effects of the drugs they're on."

Modern antipsychotics do not outperform first-generation drugs in efficacy, as revealed by the 2005 CATIE Study funded by NIMH. This extensive research found no clinical superiority for newer, often ten times more expensive, second-generation medications compared to older, cheaper versions. The study highlighted that 67% to 82% of patients discontinued the trial due to either side effects or a lack of efficacy, indicating persistent challenges in treatment. Andrew Scull plainly stated, "Are the new drugs more efficacious than the old drug? No."

For most patients, antidepressants are only marginally better than a placebo, suggesting that expectation and belief play a significant role in outcomes. Withdrawal from SSRIs is frequently described by patients as 'hell on wheels,' indicating severe and distressing discontinuation symptoms. This underscores the need for careful consideration of their efficacy and potential challenges in long-term use, especially given the strong influence of the placebo effect.

Andrew Scull observed, "For most patients, antidepressants are marginally better than placebo, but a lot of the effect is the placebo effect."

Currently, academic funding often restricts the study of crucial social issues like homelessness, despite their profound impact on mental well-being. Mental illness remains a profoundly lonely experience for sufferers, highlighting the need for comprehensive support systems beyond medication.

The future of the field depends on a fundamental shift in professional mentality, one that embraces the psychosocial dimensions of mental illness and allows psychiatrists to build careers in these vital areas.
