Black Market Treatments Lead to FDA Shift on Feline Infectious Peritonitis

For years, owners of young cats diagnosed with Feline Infectious Peritonitis (FIP) have faced a difficult reality: the disease has no federally approved treatments, and without intervention, a kitten with FIP will die. This dire situation has led some pet owners into a legal grey area, bending the law to find remedies for their beloved pets.

Remarkably, some black market treatments for FIP have proven effective. This success has even prompted the FDA to change its position, making a significant impact on the prospects for sick kittens.

Feline enteric coronavirus (FECV) is a widespread virus that affects many different animals, including cats. While any cat can contract FECV, kittens between 2 to 4 months old are typically the most vulnerable. Exposure is common, with some estimates suggesting that around 80% of cats are exposed to FECV, often through contact with other cats' feces or contaminated objects in their environment.

Symptoms of FECV are often mild, including diarrhea, vomiting, appetite loss, and brief respiratory signs, usually clearing up within a week or two as the cat's immune system takes care of the situation. Therefore, FECV itself is generally not considered a serious threat to cats' health.

For FECV to mutate into the deadly FIP, a cat must experience two unfortunate events: first, their immune system fails to initially fight off the virus, allowing it to replicate and mutate. The mutated virus is no longer confined to gastrointestinal cells and can infect immune cells. Second, these immune cells must be vulnerable to infection. Only about 5 to 10% of feline coronavirus infections result in FIP.

Once FIP develops, it is almost 100% fatal, typically affecting cats between 6 months and 1.5 years old. FIP can manifest in two forms: "wet" FIP, characterized by fluid buildup in the abdomen or chest, which can be diagnosed by testing the fluid; and "dry" FIP, which causes organ inflammation in areas like the kidneys, liver, and lungs, along with potential neurological and vision problems. This dry form is much harder for veterinarians to diagnose quickly.

If not diagnosed and treated rapidly, infected cats can develop symptoms such as fever, lethargy, organ failure, and seizures, leading to death within approximately one week.

There is a vaccine currently available to prevent FIP, but its usefulness is severely limited. The vaccine cannot be administered until a cat is 16 weeks old.

This creates a critical window of up to two months during which kittens have low maternal antibodies, making them vulnerable, but are still too young to receive the vaccine. If a kitten is exposed to FECV before vaccination, they may already be on track to develop FIP or have developed immunity, rendering the vaccine ineffective.

The FIP vaccine often proves ineffective because many cats are already on a path to developing FIP or have already become immune to FECV by the time they are eligible for vaccination, in which case the vaccine provides no benefit. Additionally, the vaccine targets a specific viral strain that accounts for only about one-fifth of infections, further limiting its broad effectiveness.

Even if a cat is kept in isolation for 16 weeks and vaccinated before any exposure, there remains a good chance they would still be susceptible to other strains. Despite these limitations, some individuals still consider a roughly 20% chance of benefit worth taking, especially when compared to the near-0% survival rate once FIP develops.

While researchers could attempt to develop a more effective FIP vaccine, such an endeavor might not be worthwhile, as it could potentially exacerbate the disease due to a phenomenon called antibody-dependent enhancement. This occurs when a virus, like the mutated FECV, can hijack immune cells.

In this scenario, if the antibodies produced by the vaccine are not strong enough to neutralize the virus, they might inadvertently transport it directly into vulnerable immune cells, acting like a Trojan horse. This could lead to the virus replicating more efficiently and consequently, making the disease worse.

Among antiviral treatments, GC376 works by blocking an enzyme essential for viral transcription and replication. However, its effectiveness has been inconsistent; a 2016 study reported it successfully treated six out of eight cats, while a 2017 study found it effective in only seven of 20 cats.

Furthermore, GC376 may be detrimental to the development of kittens younger than four months old. Currently, GC376 is not FDA-approved for FIP, meaning anyone involved in its sale, prescription, or administration faces legal risks beyond the cat's health. Despite these challenges, the manufacturer is actively seeking FDA approval.

Another potential treatment option being considered for FIP is the popular antiviral Remdesivir, also known as GS-5734. This drug is already FDA-approved for treating COVID-19 in humans.

Remdesivir functions as an antiviral by mimicking a nucleotide building block that viruses use for replication. When Remdesivir is incorporated into the viral genome instead of a functional nucleotide, it causes a disruption, effectively jamming the virus's replication machinery and stalling its ability to copy itself.

A 2025 study demonstrated Remdesivir's efficacy in treating FIP, reporting an 86% survival rate among 29 treated cats, with 25 surviving. Additional studies have shown survival rates ranging from 77% to 88%. Although Remdesivir is not FDA-approved specifically for FIP, its approval for other uses allows veterinarians in the US to legally prescribe it off-label for FIP treatment.

Despite Remdesivir's effectiveness and legal off-label availability, its high cost makes it difficult for many cat owners to afford and for veterinarians to obtain. This financial barrier and limited access have driven people to seek alternatives on the black market.

While the modest success and risk of adulteration have deterred some from sourcing GC376 and Remdesivir through illicit channels, another drug has attracted a larger black market following.

The drug that gained significant traction in the black market is GS-441524, which is the main compound Remdesivir transforms into within the body. This drug bypasses an intermediate step, potentially making it more direct and effective in reaching its target in cats. It operates on the same principle as Remdesivir, jamming the viral replication process.

A study published in the journal "Viruses" reported that all 18 cats with FIP treated with a drug containing GS-441524 as the active ingredient survived. While other studies show slightly lower but still impressive survival rates of 75% to 80%, the consistent efficacy has made GS-441524 the cat community's "dirty little secret." Since veterinarians could not legally prescribe it, an underground network of Facebook groups and overseas suppliers began selling it in unlabeled vials, often for thousands of dollars.

Driven by desperation, cat owners established an underground network to procure the antiviral drug GS-441524. Facebook groups served as anonymous forums where owners could request the medication and connect with suppliers, who often sourced the drug from manufacturers primarily in China and distributed it in unlabeled vials. Dosing advice and treatment protocols were shared through direct messages to circumvent legal accountability.

This method of treatment carries substantial risks due to the complete absence of quality control and professional oversight regarding dosage or concentration. Furthermore, there was no guarantee that buyers would even receive a product after sending significant amounts of money to unlicensed strangers. Despite these dangers, many owners continued to work with their veterinarians for monitoring and supportive care, and a significant number of cats recovered, increasing the drug's popularity among vets and owners alike.

In a 2024 statement, the FDA unofficially gave its blessing to the antiviral treatment for FIP, indicating that it would not pursue legal action against anyone prescribing or using it for FIP. This move, while not a formal approval, represents a significant win, as veterinarians can now safely obtain the medication from pharmacies, effectively eliminating the need to resort to the black market.

Despite the FDA's apparent readiness to approve the drug, formal approval has been stalled because the company holding the patent has not submitted it for approval as a veterinary medication and appears to be in no hurry to do so. This situation leaves many kittens with bleak prognoses a legitimate chance of survival without the associated risks of the black market.

While individual antivirals offer significant hope, researchers are increasingly advocating for the use of combination therapies to treat FIP, suggesting that combining different treatments may yield even better outcomes. A 2025 study proposed that using GS-441524 followed by stem cell therapy provided the best results for affected cats.

Another study investigated a combination of GS-441524 and GC376. After four weeks of this combined treatment, only one out of forty-six cats died, achieving a survival rate of practically 98%. These results suggest that combining therapies could be the most effective solution for FIP, highlighting the ongoing efforts to find definitive treatments despite patent hurdles.
